Outpatient visit
New patients

Your first visit
- Fill in the A pplication Form and submit your insurance card and Publicly Funded Medical Recipient Certificate, etc. (if you have one) to the New Patient Counter (No. 3).
- You will be given a Patient ID Card, Numbered Ticket , and Information Sheet .
- Submit your Numbered Ticket and Information Sheet to the counter of the department where you will be examined.
If you have a referral letter
- Bring your referral letter, insurance card, Patient ID Card (if you have one), Publicly Funded Medical Recipient Certificate (if you have one), etc. to the Referred Patient Counter.
- You will be given a Patient ID Card, your referral letter, Numbered Ticket, Information Sheet , etc.
- Your Numbered Ticket and Information Sheet will be confirmed at the counter of the department where you will be examined.
- When your number is displayed on the screen , enter the Waiting Area.
- When the physician is ready to see you, you will be called to the Examination Room.

If you have visited Hekinan Municipal Hospital before
- You must present your insurance card each month, so please bring it to the Insurance Card Confirmation Counter (No. 7).
- Insert your Patient ID Card into the Established Patient Check-in Machine.
If you have lost or forgotten your Patient ID Card, you can check in at the Established Patient Counter (No. 4). - You will be issued an Information Sheet and Numbered Ticket.

Blood testing or imaging only; injections, intravenous drips, or nutrition consultations

- You must present your insurance card each month, so please bring it to the Insurance Card Confirmation Counter (No. 7).

- Touch the applicable button on the Established Patient Check-in Machine.
Blood testing: Central Blood Collection Room
Imaging: Imaging Room
Injection/IV: No. 2 Treatment Room
Nutrition consultation: Nutrition Consultation Room - You will be issued an Information Sheet and Numbered Ticket.
- Bring your Patient ID Card, Information Sheet, and Numbered Ticket to the counter of the department where you will be examined.
Payment
- After your examination and testing, bring your Information Sheet to the Payment Counter (No. 5).
- You will be given a Payment Number Ticket.
- When your payment number is displayed on the screen, please pay using the A utomatic Payment Machine.
If you have any questions about how to operate this machine, please ask a staff member.
Medical certificates, etc.
- If you require a medical certificate or other physician’s certificate to submit for worker’s compensation or to an insurance company, please fill in the Document Request Form and bring it to the Main Information Counter (No. 1).
- You will be given a Document Request Form (Patient Copy).
- On the day you come to pick up your certificate, pay the fee at the Payment Window (No. 6) or Automatic Payment Machine.
- Present the receipt you were issued at the Main Information Counter (No. 1).
Urgent care
Urgent care
A. Seriously ill patients are given priority treatment, which may result in a longer waiting time.
B. We provide emergency testing only at nights and on holidays.
The medical examinations offered on weekdays may not be available. If you have experienced the same symptoms for several days, you must visit during the examination hours on weekdays. Please note that medical certificates cannot be issued.
C. We can prescribe one day’s worth of medicine only as an emergency measure.
D. For urgent care, the physician on duty may examine patients outside of his/her field of specialty.
E. Hekinan Municipal Hospital is a designated training hospital, so medical residents may provide diagnoses and treatment as part of urgent care.

- Tell the nurse on duty your name, age, symptoms, injury severity, etc.
- Bring your insurance card and other required items to the hospital.

Inpatient treatment
Inpatient treatment procedures
- Come to the Main Information Counter by 10:30 a.m. on the day of admission. After, bring any medicines you are currently taking, medicine notebook, or medicine information form to counter No. 8 (By 10:00 a.m. for the 3F Ward only).
- Submit your Patient ID Card, insurance card, Publicly Funded Medical Recipient Certificate, etc. For emergency inpatient treatment, submit these to the Ward Admission Desk. If you have an appointment on a holiday, follow the instructions you were given in advance.
- Submit the Inpatient Application/Pledge and other documents to the Nurse Center in the ward where you will be admitted.
- You will be given a wristband to identify you.
- Tell the nurse if you would like a private room.
